Monday Morning Kids Time Enrollment Form

Please complete a form for all children in your family who will be participating in Wednesday Night Life. Enrollment includes Childcare for Infants-Preschool; Club Rock for Kindergarten-4th Grade and Roots for 5th & 6th Grade.
